The problem
A clinical visit generates far more signal than ever reaches the medical record. The physician is either present with the patient or accurate in the notes, and the system quietly asks them to choose. What gets lost is not just detail. It is the context that makes the next clinician’s decision a good one.
What we did
TODO: the sequence of decisions, in order. What was the state when you arrived, what did you choose to build first, and what did you deliberately not build? The credibility of a case study lives in the things you decided against.
What changed
TODO: the outcome. If the engagement predates good measurement, say so plainly and describe the qualitative change instead. A case study that admits what it cannot measure reads better than one that rounds up.
